2008 Spring Soccer Registration

Registration is currently closed unless you are in the age brackets listed on our home page. 

   NO REGISTRATIONS WILL BE ACCEPTED AFTER JANUARY 31.

 All Registrations will be located in the basement of the Tomah City Hall, 819 Superior Ave.

Registration Fees and Uniform information for the 2008 Spring Soccer Season                                                                                                                                   

 

Mail-In Registration is permitted but registrations must be received no later than, January 19th, 2008.  Mail forms and fees to TYSA, Po Box 233, Tomah, WI 54660.

 

To pre-register, print the registration form and mail the fees and registration forms to the address on top of registration form. Also, if uniform(s) are needed print the uniform "Order Form" and send this form with the registration to the above address.
